隨著樂齡人口增加,長者面對身體變化同時要維持健康活力,飲食攝取選擇扮演重要角色,最常見的問題包括:牙齒咀嚼、肌肉肌力流失、食物與烹調的選擇與製備、慢性病患的飲食須知,如何讓營養均衡融入生活中?

講師一開始即介紹「肌少症」議題。她指出:肌肉與肌力會隨年紀而逐漸減少,而隨著年齡老化,肌肉和肌力流失速度都加快,許多老年人容易有肌少症的問題,這因為許多年長者營養攝取不足,出現肌少症的問題;老人家因為「牙口不好」,咀嚼能力慢慢變差、咀嚼不易,因此愈來愈不愛吃肉、普遍都有蛋白質攝取不足的情況。
肌少症的檢測大致有:握力量測、肌肉質量測量、身體功能表現評估,可以利用毛巾不易擰乾、或目視坐在椅子上需要扶手才能站起來、過馬路時常無法在轉成紅燈之前通過路口、直線走6公尺測秒數若小於每秒0.8公尺等方式檢查年長者的肌少症情形,一旦發現有肌少症的現象,就應透過適當的營養補充、規則正確的運動可以逆轉衰弱症,賴營養師建議年長者應該多多運動、強化體能,使肌肉變強壯,同時要攝取足夠的蛋白質,預防肌肉流失。
此外,調查發現:台灣老人有飯吃超過、奶喝太少、蔬果缺乏、堅果常沒吃、油脂偏低等飲食攝取不均衡現象。年長者要如何吃才健康呢? 賴營養師介紹新版的國民扇型飲食指導原則:(1)五榖根莖類: 3-6,(2)碗奶類: 1-2杯,(3)蛋豆肉魚類: 4份,(4)蔬菜類: 3碟,(5)水果類: 2 個。她指出每天都要維持足夠的熱量,不要隨便打發一餐,可以將全穀雜糧加入白米飯一起煮、不愛吃肉可以改吃蒸蛋/水煮蛋/滷蛋或豆製品、把青菜加在湯裡煮爛一點。她並以清楚的圖示舉例說明體重60公斤的老年人應如何攝取足夠的營養素。
此外,她特別強調年長者應適當補充維生素D,建議應從乳類乳製品(優格、起司、優酪乳、堅果、芝麻、小魚乾、黃豆製品(豆腐、豆乾)藻類和秋葵芥藍菜等深綠色蔬菜補充足夠鈣質,而有咀嚼困難的問題則選擇軟硬適中食物,並把食物切碎、攪爛、煮「腍」或加上肉汁進食,若味覺減弱、可選用天然調味料如薑、葱、蒜、果皮等提味,每天喝2000cc的水,吃飽後應從事輕快運動,並且以少量多餐,以點心補充營養,以豆製品取代部份動物蛋白質等方式維持均衡營養攝取;此外,要適時補充維生素B群,輪流將多元不飽和脂肪(如玉米油、葵花油)和單元不飽和脂肪(如橄欖油、花生油)換著吃,少加鹽、味精、醬油、少辛辣,以維持健康飲食及良好生活形態。
